Record Books
4-H Record Books are an educational tool that help youth to REFLECT on their learning, develop LIFE SKILLS of planning and record keeping, and receive RECOGNITION of their growth. We encourage all youth to complete a 4-H Record Book each year.
4-H Record Books are a REQUIREMENT of the Green County 4-H Member in Good Standing Policy (pg. 15) for youth in 3rd grade and older. Youth wishing to be eligible to represent the Green County 4-H Program as a participant of the 2027 Green County Fair, at sate of national 4-H contests that require county authorization, or to be eligible for recognition from Green County 4-H in the form of a county 4-H award or 4-H scholarship MUST complete a 4-H Record Book on the 2025-2026 4-H year.
NEW for the 2025-2026 4-H year, we have a unique 4-H Record Book specially designed for each of our 4-H age groups. The grade levels are based on the 2025-2026 school year.

Online Form – “Make a Copy” of the Google Doc, fill it out on the computer, and email to your 4-H Club Organizational Leader
- Open the document
- Click “File” in the upper left
- Click “Make a copy.” This will create and open a copy of the document that you now own, can rename, and edit in Google Docs.
Printed Form – Print off your own form or pick up a printed form at the Extension Office. Then fill out the form by hand and deliver to your 4-H Club Organizational Leader.
